Our applications are now ONLY accepted through the Internet, via email. This application can be filled out on your computer and printed at any printer. Please make sure you haveAdobe Acrobat Reader.

Video Résumés

Sarrell Dental and Eye Centers welcomes video résumés. If you would like to submit a video résumé with your application, please attach it with your résumé and application form as listed in the instructions below. Please make sure that your email is no larger than 10MB in total. Emails larger than 10MBs may not be received properly.

The video résumé format :
The video résumé should not be longer than 2 minutes. The video can be edited in the manner that seems most feasible. Please submit video résumés in either of the following video formats

• .mpeg
• .mpg
• .swf
• .wmv
• .avi

Applicant consideration will not be subject to the submission or lack of submission of a video résumé.

Application Submission

Once you have completed your application please SAVE your file onto your computer desktop. Email the completed application and your résumé as an attachment to:

The email subject line should say:

“Employment Application for Your Name”
 i.e. “Employment Application for John Doe”.
